Application Process
APPLYING for a dog does not mean you will have it tomorrow!
NO Colorado Service Dog will be placed with its handler till a
Contract Terms of Service is signed by applicant and one of the two
Colorado Service Dogs Chief Executives.
- All Candidates must agree and meet the conditions set forth by Colorado Service Dogs, Inc.
with regards to service dog ownership.

- Colorado Service Dogs, Inc. provides information about the program and answers any questions.
- The candidates fill out the following applications and provide supporting documentation
and return it to Colorado Service Dogs, Inc..
Complete applications consist of the following documents:
Application, Conditions of Application, Waiver Release, Medical Release & C.A.P.S Program
If the child is under 18: A letter is required from the parents of the minor showing
they agree with and support the decision to get a service dog and agree to assume the financial responsibility related to obtaining and caring for a trained service dog.

APPLICATION PACKET
Completed paperwork can be received by fax, email (scanned in) and by postal.
Please make sure all documents are signed before submitting.
- The in person interview, which is between 1-2 hours in length, is usually held in the applicant's home with one or two representatives of Colorado Service Dogs, Inc. and a Organization dog or dogs.
A 24 hour cancellation notice is required if an applicant must cancel an appointment with Colorado Service Dogs, Inc.
- At the point when a contract is to be signed, CAPS will be discussed (if it has not been previously). Colorado Service Dogs, Inc will attempt to be flexible in working with the recipient.
- Each recipient is responsible for the full amount quoted by Colorado Service Dogs. That is the cost to train and place a Service Dog with a candidate with a disability.
Colorado Service Dogs, Inc. will provide fund raising or sponsorship ideas to raise money for the fee. Please contact us in regards to these opportunities.
- Finally, a meeting will be held. This meeting will give families the opportunity to prepare
themselves for the next stages in the process. Final acceptance is determined following this interview,
and the search for an ideal dog begins!
Once an ideal dog is matched with a recipient, the training begins to customize the dog to the recipient’s needs. As an average, putting in an hour or so every day, doing the assignments, training could take as little as six months; some have taken 18 months, others over 2 years. The length of training is determined by many factors, including but not limited to, how often the recipient is able to meet with the trainer, dedication to completing homework assignments and the complexity of training needed for each recipient
FINDING the right dog for you is an important process and you
should expect the process to take some time.
