Application Process
APPLYING for a dog dose not mean you will have it tomorrow!
1. Candidate must agree and meet the conditions set forth by Colorado Service Dogs,Inc.
in regards to service dog ownership.
2. The candidate contacts Colorado Service Dogs, Inc.

3. Colorado Service Dogs, Inc. provides information about the program, answers any questions.
4. The candidate fills out the application and returns it to Colorado Service Dogs by postal mail.
The application includes a number of components:
5. If Colorado Service Dogs, Inc. believes the applicant will benefit from a Service Dog, the candidate is contacted to arrange for a personal interview. The interview is usually held in the applicant's home
with two representatives of Colorado Service Dogs, Inc. and a organization dog or dogs. If the candidates home is at a great distance,
other arrangements may be made. The Interview Form and client waiver is completed during the
interview and the non-refundable $500 fee is received.
6. At this point, a payment schedule is discussed. Colorado Service Dogs, Inc will attempt to be flexible in working with the recipient.
7. Each recipient is responsible for not less than $4,000. That is the cost to train and place a Service Dog with a candidate with a disability.
Colorado Service Dogs, Inc. will provide fund raising ideas to raise money for the fee.
8. Finally, a meeting will be held, this meeting will give families the opportunity to prepare
themselves for the next stages in the process. Final acceptance is determined following this interview,
and the search for an ideal dog begins!

should expect the process to take some time.
